Institutional Biosafety Committee (IBC) Forms

The Application for the Use of Recombinant DNA in Human Gene Transfer is in the process of being converted to a fillable PDF document. (The revised Certificate of Registration is already posted). We appreciate your patience as the rest of the conversion process is completed. Hard copies of IBC and other Research Integrity forms are no longer available, nor will they be accepted. This eliminates the need for submission of multiple copies for human gene transfer protocols. Therefore, please disregard any wording on the form that refers to submitting multiple copies.

In the interim, for the Application for the Use of Recombinant DNA Molecules in Human Gene Transfer, simply convert the Word document into a PDF document, combine all other attachments associated with the submission into a single, bookmarked PDF file and submit via email to: ibc@med.cornell.edu. Attachments that are too large for the e-mail server can be sent via the WCMC File Transfer System at http://transfer.med.cornell.edu to Lauren Odynocki, IBC Administrator, at lao2003@med.cornell.edu.
